On occasion, representatives from Freshwater Church wish to photograph and/or videotape Freshwater Kids in connection with church programs, project, or events.

In order to release photographs or video footage and post on our website and/or social media site, we need your permission.  To give your consent, please complete the form below.

I am the parent/guardian of the child(ren) listed below.

I give permission for my child(ren) listed above to be photographed and/or videotaped by those representing Freshwater Church.  I authorize the use and reproduction by Freshwater Church of any and all photographs and/or videotapes taken of my child(ren), without compensation to me or my child(ren).  The photographs/video recordings shall be the property, solely and completely, of Freshwater Church.  I waive any right to inspect or approve the finished photographs or videotapes.
